TRANSITION ACCOUNTABILITY PLAN

There is a strong correlation between a well-planned transition and an offender’s successful return to the community. As the offender’s assets and liabilities are determined, a detailed Transition Accountability Plan is formulated and key mentors are identified and assigned to the offender’s case management team.

Over the period of incarceration, the offender, their families, staff and community resources work together to address past issues and to ensure the offender continues to work toward achieving their goals in preparation of release. As new issues are identified, changes occur, or the offender successfully completes a goal, the plan is modified. During the final months of incarceration, the offender is assigned to a Transitional Housing Unit which provides concentrated release preparation services. These services provide a crucial link to immediate sources of assistance within the community. Training programs occur and information and assistance are made available from the partnering outside agencies. Plans are made with external sources to ensure continuity of care upon release and to develop a coordinated effort among agencies who may be involved in one or more aspects of the offender’s plan.
